EGFR Resistance After Successive TKI Generations
EGFR-mutant lung cancer evolves under treatment pressure. Earlier inhibitors commonly select for T790M, while osimertinib changes the landscape toward C797S, bypass signalling, downstream alterations and histologic transformation. This is not a fixed sequence in every tumour.
Quick Answer
EGFR-mutant lung cancer evolves under treatment pressure. Earlier inhibitors commonly select for T790M, while osimertinib changes the landscape toward C797S, bypass signalling, downstream alterations and histologic transformation. This is not a fixed sequence in every tumour.
Side-by-side comparison
The dominant resistance question depends on which inhibitor and molecular state came before it.
| Treatment context | Selected pressure | Representative resistance routes |
|---|---|---|
| First-generation EGFR TKI | Blocks sensitising EGFR but not T790M effectively | T790M, bypass signalling, transformation |
| Second-generation EGFR TKI | Broader irreversible ERBB inhibition with wild-type toxicity | T790M and heterogeneous bypass routes |
| Third-generation osimertinib | Mutant-selective covalent binding at C797 | C797S, MET amplification, pathway mutations, transformation |
Resistance Is Selection, Not a Scheduled Mutation
A tumour can contain several subclones before therapy. EGFR inhibition removes sensitive cells and creates a selective environment in which a pre-existing or newly acquired resistant population can expand.
The next detectable alteration therefore depends on baseline heterogeneity, drug exposure, disease sites, assay sensitivity and which tissue or plasma sample was tested.
Earlier TKIs and T790M
First-generation reversible inhibitors suppress common sensitising EGFR mutations, but the T790M substitution increases the competitive barrier to drug binding while retaining kinase signalling. It became a defining acquired-resistance route after gefitinib or erlotinib.
Osimertinib was designed to inhibit sensitising and T790M-mutant EGFR more selectively than wild-type EGFR, changing both treatment and the evolutionary pressure applied to the tumour.
Osimertinib and On-Target Escape
Osimertinib forms a covalent bond involving EGFR C797. The C797S substitution removes the relevant cysteine chemistry and can prevent that covalent engagement.
The allelic relationship between C797S, T790M and the original driver can matter in laboratory and clinical observations, but a molecular result alone does not define a standard next treatment.
Bypass and Lineage Change
Some resistant tumours maintain downstream output without altering the osimertinib binding site. MET or HER2 amplification, KRAS, BRAF or PIK3CA changes and other pathway events can provide alternative signalling routes.
Other tumours undergo histologic transformation, including small-cell transformation associated with TP53 and RB1 context. Tissue sampling can be important when a lineage change is suspected because plasma DNA does not show morphology.
Key Takeaways
- ·Resistance routes reflect the treatment history and tumour's evolving clone structure.
- ·C797S is one on-target route after covalent EGFR inhibition; bypass signalling and transformation are different problems.
- ·Progression testing must retain the prior drugs, original driver and specimen context.

Frequently asked questions
What is the key idea in EGFR Resistance After Successive TKI Generations?
EGFR-mutant lung cancer evolves under treatment pressure. Earlier inhibitors commonly select for T790M, while osimertinib changes the landscape toward C797S, bypass signalling, downstream alterations and histologic transformation. This is not a fixed sequence in every tumour.
What should be kept with the result or mechanism?
Resistance routes reflect the treatment history and tumour's evolving clone structure. C797S is one on-target route after covalent EGFR inhibition; bypass signalling and transformation are different problems. Progression testing must retain the prior drugs, original driver and specimen context.
